A process and an apparatus for isolating endocrine islets from a donor organ, for example from a pancreas, and the use of the isolated islets are described. In this process, the donor organ is artificially perfused with a mixed blood substance during a perfusion phase and then immersed in collagenase fluid until the fibre material separates during a dissolution or digestion phase until a homogeneous fragment suspension is formed. The fragment suspension is centrifuged to isolate a sediment from exocrine cell material and endocrine cells, so that in an islet isolation phase the dead endocrine cell contents can be separated from the live endocrine islets. The atraumatically isolated islets can be transplanted or injected for the resumption of the normal metabolic function of an organ. IMAGE
