The responses are recorded on a card which has its face marked and has bar code identifiers on one surface. The card is scanned optically to extract the response data. The card has on its face an arrangement of boxes in which the response is placed with, if necesary, coding to facilitate the automatic identification of the box. The completed card is scanned optically to extract the response and the bar code read to identify the document.
