PURPOSE:To easily collect information on exhibition-goers and questionnaires and input these data quickly and assuredly by imparting a machine readable customer identification code which means the same code to each of a registration sheet, a research sheet, and a customer card. CONSTITUTION:As a customer identification code 6 which is provided on each of a registration sheet 3, a research sheet 4, and a customer card 5 constituting a registration form 1 for exhibition-goer, a bar code, a two-dimensional code, an OCR, an OMR or a magnetic record medium are named. These are formed by a man-impact printer, a dot impact printer, other printing means, and an information write device. By forming the customer identification code 6 simultaneously on the customer sheet 3, the research sheet 4, and the customer card 5 in a state of a single sheet of the exhibition-goer registration form 1, a customer identification code having the same meaning can be formed assuredly. Thus, information on individual exhibition-goers and information on researches can be collected.
