PURPOSE: To obtain an information recording medium and reader capable of previously checking recorded information contents. ;CONSTITUTION: This information recording medium 1 has an information recording member (CD) 3 recorded with the prescribed information, a housing member 5 housing this information recording member and an information content reading member 7 capable of optically reading the information contents recorded on the information recording member. This housing member has a housing body 5a for housing the CD and a package 5b made of a transparent vinyl-polymer for packaging this housing body. The information content reading member has a data code part 7a coded and printed with the information contents associated with the information recorded on the CD on its front surface and is freely attachably and detachably stuck onto the package 5b with an adhesive member formed on its rear surface.
