The invention relates to an integration medium for capturing, preparing, transmitting and storing recallable medical data on an individual which are available to the latter at any time. The aim is preferably to protect the filing of diagnoses and findings which are not necessarily available in digital form and may exist in the form of image, text and sound data. According to figure the aim is attained in that a patient's card system treating unit (1) integrates a PIS/KIS (patient and hospital information) system (2) and is in effective communication with a prior art card (3) of credit-card size and having a file (4) via a prior art read-write device (5), in which the PIS/KIS system (2) does not integrate the read-write device (5) and the patient's card system preparation unit (1) integrates the read-write device (5) and excludes the PIS/KIS system file (11).
