Apparatus and methods suitable for minimally invasive harvesting of internal mammary arteries suitable for blood vessels, particularly coronary artery bypass grafts, are disclosed. Generally, the electrosurgical instrument is provided with a scissors or other end effector for cutting tissue. The instrument also has mono polar or bipolar output. The bipolar blade geometry provides current flow between the blades or within the scissors of each of the blades or both. The apparatus includes at least one ergonomically positioned actuator for actuating a current flow for cutting tissue, for cauterizing tissue, or for actuating a blade or end actuator to act simultaneously or in succession, .
