Provided is the use of novel treatment of L-carnitine, acyl L-garnitine and pharmacologically acceptable salts thereof for the treatment of immune and metabolic changes in patients with AIDS and AIDS related syndromes and asymptomatic HIV seropositive patients. The dosage of L-carnitine according to the novel use of the present invention is about 6-8 g per day or equivalent amount of acyl L-carnitine or a pharmacologically acceptable salt thereof for oral administration. The dosage is 3-4 times the usual dosage for known therapeutic uses of L-carnitine.
