CARE PLAN DECISION SUPPORTING SYSTEM AND EVALUATION FORM USED FOR THE SYSTEM

PROBLEM TO BE SOLVED: To provide a system for supporting a task for simplifying the state evaluation process of a care facility user such as an aged person, and quickly and efficiently deciding a care plan. SOLUTION: A system server 10 is provided with a facility basic information database 130 for mainly storing the daily basic schedule of a care facility and a sub-schedule defined for each group of users, user information database 140 for storing the information of each user, and information processing part 110 for controlling the data input and output of those data bases. A time zone is set for the basic schedule and the sub-schedule for each user group so that mutual competition can be prevented. The state of the user is processed based on a care plan decision evaluation report generated based on an evaluated result obtained by using an ALD (daily life operation) check list and a mental side check list, and this is provided with a column in which an incentive item for each of 18 problem areas of an MDS (aged person assessment chart), the score of each occupation corresponding to this, and exponents indicating the physical and mental sides of the user are written.
