Regulating the systemic to local distribution ratio of insulin-like growth factor-1 (IGF-1) comprises administering at least one hepatic IGF-1 production inhibitor to achieve the desired effect without inhibiting body growth and/or anabolism. - Independent claims are also included for the following: - (1) specifically stimulating growth in peripheral target organs of growth hormone (GH) to more than 20-50% of the maximal effect of GH on body growth, specifically stimulating body growth or specifically stimulating anabolism in peripheral target organs of GH with a concomitant small effect on hepatic IGF-1 production or circulating IGF-1 levels, which comprises administering at least one stimulator of peripheral extrahepatic IGF-1 gene expression, optionally simultaneously with intermittent administration of GH; - (2) use of inhibitors of hepatic IGF-1 production, or of IGF-1 receptor antagonists, or of small nucleic acid molecules for inhibition of liver IGF-1 production, for the manufacture of pharmaceuticals for reducing fat mass and - (3) use of IGF-1 receptor antagonists, agonists binding to and activating the same receptors as estradiol or small nucleic acid molecules for inhibition of liver IGF-1 production or small nucleic acid molecules for inhibition of liver IGF-1 production for the manufacture of pharmaceuticals for reducing retinopathy in diabetic patients. - ACTIVITY - Anorectic; ophthalmological; hepatotropic; cytostatic.
