The instrument has a gripper with jaws (34) with free ends (14) of the ligature cord (10). It has a carrier (20) with a support (39) allowing manual handling of the instrument and a threaded shaft (22) mounted for rotation in the support. The shaft carries the gripper and a movable sleeve (24) with a ratchet (40). The ratchet can pivot to cooperate with the thread when it projects to cause rotation of the shaft when the handle is held and the sleeve moved along the shaft. A mandrel (20) can be interposed between the shaft with threads and the handle.
