The invention concerns a booklet (1') comprising a plurality of sheets bound with binding means, and a form (F') detachable from said binding means, said form (F') comprising a plurality of input zones for receiving data or hand-written information items and a removable self-adhesive tag (E). Said form (F') further comprises, within a detachable flap (V3), a first detachable fixing part (P1), a second detachable part for data input (P2), and a third detachable part (P3) for supporting said self-adhesive tag (E), and the self-adhesive tag (E) comprises a receiving part (ZT) designed to be fixed by its adhesive surface on an external support (ZE) and a linking part (L) attached to said receiving part (ZT) and designed to receive on its adhesive surface said fixing detachable part (P1) pre-detached from said detachable flap (V3) and whereto is temporarily maintained attached the second data input detachable part (P2). The invention is particularly useful for supply of landing/boarding cards.
