PROBLEM TO BE SOLVED: To allow an individual oneself to perform mental health dementia prevention, etc., by putting individual's past mental transition in a database and to enables a doctor, a helper, etc., to use the database for patient caring. SOLUTION: This system is equipped with a question template storage means 1 which previously stores question templates including changeable parts having attributes as question sentences urging a user to recollect, a communication means 2 which outputs a question sentence to the user and obtains an answer to the question sentence from the user, an answer storage means 3 which stores the answer to the question sentence in the form of a database together with the attribute of the answer, a question sentence generating means 4 which reads a question template out of the question template storage means 1 and an answer having the attribute matching the changeable part of the question template out of the answer storage means 3, generates a question sentence by applying the read answer to the changeable part of the question template having the matching attribute, and outputs it to a communication means 2, and an answer output means 5 which converts the answer put in the database by the answer storage means to specified format and outputs the converted answer.
