Intertrochanterian descending femoral osteotomy is conducted by rotating an osteotomized fragment outwards and inwards, it is medialized and descended directly under the lower edge of cotyloid cavity, in supracondylar area one should carry out femoral osteotomy and simultaneously arrange distal fragment to normocorrection of biomechanical axis of lower limb that redistributes the loading from superiolateral segment of the caput onto inferior edge of cotyloid cavity at compensation of limb shortening.
