The insert (24) has a hollow projection (46) with recessed push-through membrane (56) and an inward pointing inner skirt section (70) which ends at its free end in a sealing strip (74). The membrane is set away from the sealing strip which in turn sealingly surrounds a removal spike (80) which passes through the membrane. The inner skirt section is designed for bending out whereby it adjoins and seals the removal spike which passes through the membrane.
