An adapter (1), which passes through an aperture (3) in the cover (4) of a nuclear reactor vessel, is repaired by cutting through the welded joint (7) between the adapter coupling (6) and sleeve (2), machining a chamfered edge on the upper end of the sleeve, applying a new adapter with a chamfered coupling edge, and re-welding the components together by filling the chamfers with metal. The position of the adapter is monitored before and after welding.
