POSTAL MATTER WITH HEALTH INSURANT CERTIFICATE

PROBLEM TO BE SOLVED: To provide a postal matter with health insurant certificate for sending card-like health insurant certificates addressed to a household covering all family members and confirming the receipt of the certificates. SOLUTION: Three pieces of paper 2, 3 and 4 connected to each other through foldable parts 5 and 6 are folded back at the foldable parts 5 and 6 and at the same time, bonded so that the folded-back pieces 2, 3 and 4 can be spread open. In addition, the health insurant certification cards 14a to 14d are provided on the pieces 3 and 4 in such a way that the cards 14a to 14d are separable and address information 8 and sets 19 to 19d of insurant information covered by the health insurant certification cards 14a to 14d, are displayed in such a manner that the described information 8 and 19a to 19d are located on the same face side as when each of the pieces 2, 3 and 4 are spread open. The address information 8 is displayed in a visually recognizable manner and the sets 19a to 19d of insurant information are concealed so that their visual recognition is difficult. Besides, a reply postcard for receipt confirmation with a signature and seal affixing column is provided, in a separable manner, in a part of the piece 2 of paper by demarcating the reply postcard using a cutting line.
