PROBLEM TO BE SOLVED: To perform light phase modulation precisely with a target amount of modulation at any part regardless of the spatial frequency of a partial pattern for forming a phase modulation pattern, when performing the multiplex recording of data by a phase modulation multiplex method.;SOLUTION: A control section 28 increases a display signal level at a part having a higher spatial frequency, and reduces the display signal level at a part having a low spatial frequency, according to the spatial frequency of a pattern at each part for forming a phase modulation pattern created from a phase address code for giving light phase modulation information, for displaying the phase modulation pattern in a phase modulator 22. As a result, the phase modulator 22 corrects deviation from the amount of target modulation by the level of a spatial frequency and precisely performs the light phase modulation of reference light or reproduction light.
