PROBLEM TO BE SOLVED: To provide a delivery slip, in which privacy is protected and also the value of a delivery article can not be judged from the delivery slip during delivery when concealed information entered in the delivery slip is the value of the delivery article. ;SOLUTION: In the delivery slip, both a sticking form 2, in which delivery information 20 is entered, and a delivery form 4, in which delivery information 22 is similarly entered, are separably joined through a slit 7. A receipt form 8 of money being a consealed information-entering form, in which receipt information 23 of money including concealed information such as the value of a delivery article is entered, is overlapped on the delivery form 4 and releasably bonded by a re-releasable type bonding layer 14. Also, a bonding layer 17 is provided on the rear side of the sticking form 2 and a releasable rereleasing type bonding layer 15 is provided on the rear side of the delivery form 4.;COPYRIGHT: (C)2001,JPO
