An kava-kava (Piper methysticum Forster f., Piperaceae. Kava) extract comprising reduced contents of flavokawains, including flavokawain B, having liver injury effects and/or reducing type glutathiones which are added and having flavokawain hepatoxic antagonistic effects is disclosed, and a pharmaceutical comprising the extract and a food supplement and a healthy food comprising the same are described; the invention also disclosed a preparation for kava-kava extract comprising the following step: the crude kava-kava extract is treated with a macroporous resin, silica gel, alumina, polyamide; or flavokawain B and flavokawain A are removed with a low polarity organic solvent in aqueous alkaine solution, the aqueous solution is further adjusted to be acidic with acids and extracted by organic solvents; or reducing type glutathiones capable of antagonisting flavokawain hepatoxic effects are added to the kava-kava extract or preparation so that the content of flavokawain B is less than and equivalent to 1.0% and/or the contents of glutathiones is greater than and equivalent to 1.0% in resulting refined extract.
