PROBLEM TO BE SOLVED: To provide an envelope en bloc type application blank handling with the application blank and an envelope en bloc and a booklet-like medium.;SOLUTION: The envelope en bloc type application blank 10 is equipped with an information forming base material 11, on th surface of which an application filling up surface 11a with an entry column, a release layer 12 formed over nearly the whole surface of the rear surface of the information forming base material 11, a label base material 13 having an address printing surface 13a, on which an address column is provided, a self-adhesive layer 14 for pasting the label base material 13 and the release layer 12 together and cutting-out lines 15, which are formed in three peripheral sides or the top side and the right and left sides of the information forming base material for expressing the sticking allowance 14a of the self-adhesive layer 14.;COPYRIGHT: (C)2006,JPO&NCIPI
