PPROBLEM TO BE SOLVED: To highly conveniently and reliably authenticate a maintenance mode in a simple and easy fashion without risk of stealage of an authentication input for the maintenance mode even when a third person is present nearby nor requiring hardware such as an external device. PSOLUTION: A multi-functioned machine 10 is configured to be subjected to a prescribed maintenance in maintenance mode and a maintenance authentication device incorporated into the multi-functioned machine 10 authenticates a user and permits the user to use the maintenance mode according to the authentication result. In such a case, the user authentication for enabling the maintenance mode to be used is performed by using an authentication form 20 having a printed authentication pattern which fails to be exactly copied by copying. The authentication pattern of the authentication form 20 is scanned and read in by the multi-functioned machine 10, and thus the maintenance mode authentication device performs authentication based upon the read-in authentication pattern, so that the multi-functioned machine enters the maintenance mode when the authentication is successful.
