Systems, products, and methods for predicting changes and fracture in trabecular bone are described herein. In some embodiments, systems, products, and methods for simulating bone loss that include reading a three-dimensional image of a trabecular bone, determining a number and location of bone resorption sites, and carrying out simulated bone resorption are described. In other embodiments, systems, products, and methods for determining risk of fracture in trabecular bone that include reading a three-dimensional image of a trabecular bone; removing voxels from trabeculae portion of the image to obtain a skeletonized structure of the trabecular bone, where the skeletonized structure of the trabecular bone is represented by voxels classified according to at least one topological classification; and adding reconstruction voxels to at least voxels of a first topological classification are described.
