PPROBLEM TO BE SOLVED: To provide an extrusion-molding die and an extrusion-molding method both capable of molding a soft thermoplastic resin into a medical tube having a complex shape. PSOLUTION: The extrusion-molding die 20 for molding the medical tube 10 made of the soft thermoplastic resin and having flow passages 13a, 13b, 13c and 13d formed in the inside with an axial part placed between them comprises a die main body 21 and a pin 22 having a land part 23. A cylindrical rear side-forming space for making the molding material pass therethrough is made between the inner circumferential surface of the concave parts 26a and 27a of the die main body 21 and the outer circumferential surface of the pin 22. A tip end side-molding space having a cross section to mold the medical tube 10 is made between the inner circumferential surface of the concave part 28a of the die main body 21 and the land part 23. Further the cross section of the rear side part of the cross section of the tip end side-molding space is made an intermediate shape of the cross section of the tip end of the tip end side-molding space and the cross section of the tip end of the rear part side-molding space.
