The unit has seven daily rows/columns with three hour of day compartments. A box-shaped lid (33), folded about an axis extending along an edge of a blank (13), runs at a right angle to parallel day rows/column areas. The lid can be folded from a position covering the blank to a position releasing an opening side of the blank. The unit has a configuration of printable fields for a patient and data related to medications.
