A high frequency relay, a high frequency module, and an insert mounting adapter board for the high frequency relay are provided to prevent stress when bending terminals in converting an insert mounting type relay body into a surface mounting type relay body. A high frequency relay(51) includes a relay body(1) and an adapter substrate(52). The relay body is mounted on a substrate to convert a high frequency signal. The adapter substrate is between the relay body and the substrate to adjust impedance of the relay body. The adapter substrate has a substrate body and lead terminals. The substrate body has through holes. Each of the lead terminals has one end as a connecting unit, a middle part as a conductor pattern, and the other end as a terminal part. The lead terminals are inserted-molded with the substrate body.
