An apparatus for forming notches on an optical fiber and an optical fiber with the notches are provided to form a fine notch pattern on an optical fiber by processing optical fiber yarn through a reciprocating horizontal movement of a movable plate and a reciprocating vertical movement of a notch hook. In an apparatus for forming notches on an optical fiber, a notch forming unit(30) includes first and second horizontal frames(31,32). The first horizontal frame includes a movable axis(36), a drive unit(34) rotating the movable axis, a movable plate(37) which horizontally moves according to the rotation of the movable axis and is screw-coupled with the movable axis, a notch hook(38) reciprocating vertical motion, and a fabrication unit(38a) combined with the lower part of the notch hook. A notch pattern is formed on optical fiber yarn guided by a guide unit of the second horizontal frame by a reciprocating horizontal motion of the movable plate and a reciprocating vertical motion of the notch hook.
