METHOD OF PERMANENT LUNG ROOT BLOCK DURING OPERATIVE PROCEDURES ON CHEST ORGANS

FIELD: medicine.;SUBSTANCE: invention relates to toracal surgery, anesthesiology and reanimatology, and is intended for prevention pain development during operative procedures on chest organs. Essence of claimed method is in performing thoracotomy in 4 and 5 intercostal region with further mobilisation of lung root elements, resection of lobe, segment or total extraction of lung. Then all tissues of chest wall are punctured with puncture needle 1.4 mm in diameter in 3 intercostal region along parasternal line and polychlorvinyl catheter is inserted according to Seldinger's technique. Mediastinal pleura is cut 4-5 cm from upper edge of lung root elements, cut being 0,5 cm long; distal end of catheter is installed in adipose tissue of mediastinum 1.5-2 cm from upper edge of lung root elements; hermiticity around distal end of catheter is created by applying purse-string suture on mediastinal pleura. After that proximal end of catheter is fixed with ligature thread to skin, thoracotomy wound is sutured, and 1% solution of lidocaine is introduced through installed catheter.;EFFECT: obtaining more stable indices of hemodynamics, reduce neuroendocrinal stress of organism reaction, level of blood cortisol, abate or eliminate post-operative pain syndrome in early post-operative period.;2 tbl, 1 dwg
