FIELD: medicine; surgery.;SUBSTANCE: skin platform is cut out over a distal part of trapezius muscle with extending over fascia of broadest muscle of back laterally. The flap includes fascia of broadest muscle of a back and inferior portion of trapezius muscle. In elevation of musculofasciocutaneous flap at mesoscapula level, a.dorsalis scapulae is visualised with crossing muscular branch under greater rhomboid muscle. Small rhomboid muscle is dissected. Inlateral direction from m. levator scapulae, a. cervicalis superficialis entering into trapezius muscle is visualised. A turning point of flap with two feeding vessels is m. levator scapulae. In that specific case, to cut out a skin platform, reference points are used in addition, including: thoracic spine, medial edge and inferior angle of scapula.;EFFECT: more stable blood supply of flap, possibility to replace deep and through defects.;2 cl, 1 ex, 2 dwg
