The instrument (20) has a rod-shaped handle (22) comprising an arm (24) arranged at a distal end of the handle. An axis of the arm is inclined with an axis of the handle at an angle between 90 and 180 degrees. A cutting unit (26) e.g. laser beam cutting unit, is fixed at a free end of the arm and arranged in front of the arm for epispastic cutting. A cold light mechanism is integrated in the arm for illuminating an area of the cutting unit. The cold light mechanism comprises a light conductor connected with the handle. An independent claim is also included for a method for operative treatment of hypertrophic obstructive cardio myopathy of sub-aortic septum.
