A system for monitoring the health related disorders of the patients, comprising: a plurality of remote monitoring stations (114), each being adapted to receive the health related data of a patient belonging to a respective patient; and a computer network (100), comprising a database containing accumulated health-related data pertaining to health-related disorders and their treatment, and at least one data access device, adapted to provide access to the provider of health care to said computer network and said database, said computer network (100) being adapted to receive said data related to the health of the patient of said remote monitoring stations (114); said remote monitoring station (114) being adapted to receive from a respective patient said data related to the patient's health in relation to the integration of a selected one of said treatment programs in the patient's lifestyle, the data comprising at least one of the questions concerning health or treatment and answers to questions concerning health or treatment, generated using said remote monitoring station (114); said computer network (100) being configured with assessment tools to allow a healthcare provider to assess said data related to the patient's health to determine the patient's progress in the selected treatment program and whether the information related to the treatment program selected needs to be transmitted to the patient; characterized in that said computer network (100) is also adapted to establish a treatment program for said patient based on their data related to the health of the respective patient and said accumulated data related to health, and to review said accumulated data related to health based on data related to patient health.
