The invention is utilized in neurosurgery and tissue engineering of organs for the replacement of nerve tissue defects of the brain and spinal cord of mammals during reconstructive neurosurgical operations.  The object of the invention is to develop a tissue-replacing artificial cell-biopolymer neuroendoprosthesis system (ACBP NEPS) for plastic surgery of defects of the nerve tissue of the brain and spinal cord, making it possible to stimulate regeneration and growth of damaged nerve cell axons, and also to develop a method for preparing the ACBP NEPS and a procedure for performing a reconstructive neurosurgical operation using the ACBP NEPS.  The ACBP NEPS comprises a resiliently elastic cell-biopolymer biologically active mass, prepared from a heterogeneous collagen-containing matrix for implantation and a biocomposition of cellular preparations of various types of autologous cells of the patient.  The biocomposition contains, in NaCl solution, neural stem cells (NSC), neuroglial sheathing cells (NGSC), endothelial cells with the CD34+ marker (EC) and purified mononuclear cells (MN) with the following ratio of components (in parts according to the number of cells): NSC – 0.8-1.2; NGSC – 1.6-2.4; EC – 4-6; MN – 4000-6000.  The method for preparing ACBP NEPS includes perfusing a biocomposition of cellular preparations of various types of autologous cells of the patient into a heterogeneous collagen-containing matrix for implantation to produce a resiliently elastic cell-biopolymer biologically active mass.  The procedure for performing a reconstructive neurosurgical operation for the replacement of defects of the brain and/or the spinal cord and/or the vegetative nervous system of a mammal includes implantation of the ACBP NEPS into a nerve tissue defect.
