METHOD OF COVERING WOUND DEFECT OF PILONIDAL CYST EXCISION AT STAGE OF ABSCESS

FIELD: medicine.;SUBSTANCE: invention relates to medicine, namely to coloproctology. Anatomic type of pelvis is determined. After first operation in gluteral-sacrpcoccygeal area defect is cut out of triangular shape with angle in apex directed to sacrum, in case of narrow pelvis 70-80°, in case of intermediate type of pelvis 81-90°, in case of wide type of pelvis 91-100°. Plastic restoration of triangle-shaped defect is performed. Congruent to defect cutaneous-subcutaneous-fascial-muscular transplant based on gracilis muscle of hip, including distal end with located above it cutaneous-suncutaneous-fascial plate of triangular shape with angle in apex 70-80° in case of narrow type of pelvis, with angle in apex 81-90° in case of intermediate type of pelvis, with angle in apex 91-100° in case of wide type of pelvis. Exposure of proximal end of muscle leaving intact vascular-nervous pedicle is performed, muscle is separated from pubic bone. Transplant is turned at angle 180-190° in frontal plane in area of vascular-nervous pedicle in composition of fascial sleeve and its distal end is brought via subcutaneous tunnel to defect of tissues in gluteral-sacrpcoccygeal area, after which layer-by-layer fixation is carried out.;EFFECT: method allows to achieve revascularisation of plasty zone, prevents suppurative complications, reduces probability of pilonidal cyst recurrence.;3 ex, 2 dwg
