PROBLEM TO BE SOLVED: To provide a high-quality floor slab form for a side ditch cover body, which can reduce construction costs including a transportation cost, by enabling a limited number of workers even with no high-level skill to perform construction work in a short period of time without being affected by weather and an air temperature, and which is excellent in earthquake resistance and durability.;SOLUTION: This floor slab form 1, which is made of a metal steel plate with rust resistance and corrosion resistance, includes a bottom plate portion 3, and first and second side wall portions 6 and 7 and third and fourth side wall porions 10 and 11, which are provided at the peripheral edge of the bottom plate portion 3. The inside of the form includes a plurality of reinforcing steel materials 13, and a plurality of auxiliary reinforcing steel materials 14 which cross the reinforcing steel materials 13; the third and fourth side wall porions 10 and 11 include first and second wall plate portions 18 and 19; and a plurality of radial reinforcement groups 20 and a first horizontal reinforcement 21 are provided between the first and second wall plate portions 18 and 19.;COPYRIGHT: (C)2011,JPO&INPIT
