A method for providing distributed care plan management may include receiving,at a service platform, information associated with a care plan item for apatient in whichthe information is received from a health system entity among a plurality ofhealth systementities with which the service platform is capable of communicating. Themethod mayfurther include storing the information in association with a care plan of thepatient, andproviding delivery of at least a portion of the information to a differententity with selectedview parameters for rendering the portion of the information being determinedbased onan identity of the different entity. A corresponding computer program productandapparatus are also provided.
