According to the invention, vital data are continuously recorded and/ormeasured for theremote diagnostic monitoring and support of patients. Said data areinterpreted by meansof signaling technology and evaluated with regard to the progression thereofand thecontext in which said data were recorded/measured. A link is made from thevital data toa patient profile and evaluated based on threshold values in order torecognize deviationsin the condition of a patient from a previously determined target condition. Acategorization is done as to whether an interaction with the patient isrequired or notrequired in a specified time frame based on the evaluation.
