The instrument has a shield (9) movable from an insertion position into a working position by a supply pipe (3) i.e. dual-lumen pipe. The shield is located in a flexible protective pipe (1) in the insertion position. The shield is located partially outside the protective pipe and spread above the protective pipe in the working position. The shield with a central base is held at the supply pipe. A peripheral edge (11) of the shield encloses a chamber (13) in the working position. The chamber is opened towards a shield side adjacent to a patient and receives substances to be supplied. The shield is designed as a plastic mold body from a foam material or a floating material.
