PPROBLEM TO BE SOLVED: To easily perform, in the case where a paper document printed out and output once is lost or damaged, the restoration of the paper document. PSOLUTION: Document data comprising a plurality of pages are stored (S101), and for each page of the document data, setting is performed to define the present page and the other page as a restorable range. Then, a QR code (two-dimensional bar-code), including the document data within the set restorable range and page information representing pages of the document data, is created for each page the document data (S102-S115), and the created QR code is disposed at a predetermined position on each page of the document data (S116). Afterward, while the document data in which the QR code is disposed for each page are printed out and output to a recording medium (paper) for each page, and when it is impossible to perform restoration from the QR code, restoration is performed from the stored document data.
