In the present invention, resin is supplied from several locations at the perimeter of a cavity provided in the opposing surfaces of a first mold and a second mold. Resin is fed to channels (37a, 37b, 37c) in a fixed template (33) via a channel switching piece (36). Resin is supplied to three locations at the perimeter of the cavity from the ends of the channels (37a, 37b, 37c). The resin flows from the three perimeter locations towards one gate forming part. With only a slight gap, a wide surface area is achieved, and resin flows uniformly even in a cavity having a curved shape.
