PROBLEM TO BE SOLVED: To provide a form apparatus for molding a concrete product such as a side-ditch block, which facilitates removal of a form and having a structure of good operability.;SOLUTION: The form apparatus for molding the concrete product includes a lifting mechanism 70 disposed on the front side of a base 20 to lift an inner mold 60 while maintaining the parallelism of the base 20 and inner mold 60. The concrete product is lifted by a lifting means such as a crane so as to be at the same height as or higher than a form removal position (a height position to draw the concrete product out of the inner mold), and in the lifted state of the concrete product, a rotating shaft 90 is turned to put the inner mold 60 in narrowing operation to thereby form the clearance with the concrete product. The concrete product is drawn out of the inner mold 60 while the rotating shaft 90 being positioned above the lifting mechanism 70 through a connecting member 80.;COPYRIGHT: (C)2011,JPO&INPIT
