FIELD: medicine.;SUBSTANCE: diagnostic rotation angiography with 3D reconstruction of the carotid artery is performed from a preliminary transfemoral or other peripheral approach. That is followed by an X-ray controlled derivation of a globule of the clavisternomastoid muscle at the level of the IV cervical spine. The skin pre-treated with novocaine is punctured at 1.5-2 cm from the level of an upper edge of the thyroid cartilage, and a needle is delivered anteroposteriorly a little bit laterally at 40° degrees to the neck surface from an anteriormedial approach. The needle approaches a level of the internal carotid arterial pulsation. An anteromedial wall of the anterior carotid artery is punctured under regional anaesthesia according to the Seldinger's procedure at the level of the IV cervical spine. A correct position of the needle is determined under X-ray control and by an income of pulsating red blood. An introducer is further inserted to deliver an emboloprotective system into the internal carotid artery at a depth of 30 mm. The introducer is anchored to the neck skin.;EFFECT: method enables the adequate blood flow recovery, prevents acute intra- and postoperative impaired cerebral circulation, makes the patients feel better and be adapted fast after the operation and involved into the active lifestyle.;1 ex, 2 dwg
