The template has guide elements (2a-2n), where each guide element includes a groove of conjugate shape of a tube (T) for allowing introduction and withdrawal of the tube. A manually bendable metal strip (1) includes a longitudinal edge for securing the guide elements having a conjugate profile of a longitudinal profile of the tube such that the groove of each guide element attached on the metal strip is centered on a longitudinal axis (X) of the tube. The metal strip includes support elements (7, 8a-8m) to maintain and stiffen the metal strip in its bent state.
