The system has a housing (1) for self-capturing, editing and storing answers to a set of questions, where the housing is arranged with a set of keyboard input keys, a LCD screen, and an electrically EPROM for recording data related to asked questions, and responses provided by a user. A RFID stage receives and transmits the data. A remote RFID terminal (2) loads data specific to asked questions and communicates with a portable computer (3). The housing includes reading and/or writing units for cooperating with the terminal in a wireless manner.
