An extracorporeal blood filtration system (100) to remove unwanted molecules from a flow of blood from a patient, comprising: a first hemofilter (13) to raise hemoconcentration of the blood in a blood flow filter; a first replacement fluid (17) to supplement blood flow filter; one second hemofilter (19) for filtering unwanted molecules supplemented blood flow; and a second replacement fluid (23rd) to supplement flow from the second hemofilter output for return to the patient.
