Provided is a questionnaire system which makes users feel the enjoyment, inspiration and the like as a form of entertainment, and which can impart a response method which is both intuitive and which appeals to the senses. The present invention is a questionnaire system that gathers response information, said system comprising: a questionnaire response device equipped with a display unit that displays response results and a physical sensor; and a server device which can be connected via a communication line. The questionnaire response device comprises: a response information acquisition means which acquires response information concerning the physical operation with respect to the questionnaire response device and concerning the external conditions, a display means which displays the response results of questionnaires according to the response information acquired; and a response information transmitting means which transmits the aforementioned response information to the server device. The server device comprises response information receiving means that receives response information transmitted from the questionnaire response device, and a response information storage means that stores the response information received.
