An ultrasound elasticity image based method and system for elastic strain evaluation, the method comprising: reconstructing an ultrasound elasticity information image according to an ultrasound elasticity image and a B mode image, the ultrasound elasticity information image containing elastic strain information (102); performing automatic segmentation on lesion areas in the B mode image (104); performing automatic definition of soft and hard areas in the ultrasound elasticity information image (106); performing hardness characteristic extraction on the automatically segmented lesion areas and peripheral areas thereof according to the automatically defined soft and hard areas (108); and performing elastic strain evaluation on the lesion areas and the peripheral areas thereof on the basis of the extracted hardness characteristic and according to pre-set grading criteria (110). The automatic image segmentation in the present evaluation method avoids the subjectivity of manual segmentation and improves the accuracy of elasticity characteristic extraction.
