A catheter (32) is disclosed that comprises an expansion element (34b) comprising a plurality of electrodes (36 1-N). The expansion element is configured to expand within a heart chamber so that at least a portion of the plurality of electrodes contact a wall of the chamber and a plurality of electrograms can be determined at substantially the same time in response to an electrical perturbation. The catheter may comprise pacing electrodes (82 1-M) to provide the perturbation. An activation map of the chamber can be determined based on the plurality of electrograms. Based on the activation map, a location and a shape of a mechanism of action of an arrhythmia can be determined and a treatment plan can be developed. For example, the treatment plan can include guiding an ablation of the mechanism of action.
