A system and method for chest x-ray tomosynthesis that improves spatial resolution in the direction in which the chest is flattened for examination. In addition to X-ray data acquisition of a 2D projected tomosynthesis image ETp1 over a shorter source trajectory similar to the known chest tomosynthesis, a complementary 2D image ETp2 is taken over a longer source trajectory and two sets of projected images are taken for the thickness of the chest. A chest slice image ETr that exhibits expanded spatial resolution, including direction, is processed. Additional features include flattened chest CT, multimode tomosynthesis, and shielding from the patient's mobile equipment in an upright patient.
