TECHNIQUE OF TAILORING GRAFT FOR DESCENDING COLON ONTO PERINEUM IN ABDOMINAL-ANAL RESECTION OF RECTUM

The technique of tailoring the graft for descending the colon onto perineum in abdominal-anal resection of the rectum comprises inferior-medial laparotomy, mobilization of the sigmoid colon, the cut of the inferior mesenteric artery below the origin of left colon artery or first sigmoid artery, demucosation of the anal canal on the side of perineum, the resection of rectum with cancer, the descending of the colon graft onto the anal canal and its fixation to perianal skin, the drainage of presacral space through contra-aperture on the left buttock. The graft is tailored from the sigmoid colon by ligation of sigmoid arteries. The inferior mesenteric vein is separated and ligated at the level of duodenal-jejunal transition. The distal area of the tailored graft is marked by ligating the lumen.
