A pharmaceutical composition administrable from a metered dose inhaler, comprising: a suspending medium comprising a pharmaceutically acceptable propellant; a plurality of active agent particles comprising an active agent selected from glycopyrrolate and formoterol and any of its salts, esters, enantiomers, mixtures of enantiomers or solvates; and a plurality of respirable suspension particles that are perforated microstructures comprising DSPC (1,2-diesteroyl-sn-glycerol-3-phosphocholine) and calcium chloride, and wherein the plurality of active agent particles is located in conjunction with the plurality of particles in suspension within the suspension medium forming a cosuspension.
