An orthopedic implant (10, 40, 100) has a plate (12, 42, 120) and bone screws (80, 80a). Configured bone screw pockets (19, 23; 49, 51; 190, 230) of the plate provide variable angling and positional locking of a received bone screw relative. Each bone screw includes distal threads (82), a neck (88), and a dual threaded head (85). The plate has a first bone screw pocket (19, 49, 190), a second bone screw pocket (23, 51, 230), and a strut (16, 46, 160) extending between the two bone screw pockets. The bone screw pockets are at least generally cup shaped for receipt of a bone screw. The lower periphery of the opening of each bone screw pocket has an arrangement of configured prongs (26, 260) with configured openings (27, 270) to receive threading on the underside of the dual threaded head of the bone screw. The prongs (26, 260) can preferably, but not necessarily, deform slightly and grab onto the threading of the bone screw head to lock the bone screw in the bone screw pocket. Each prong can act independently. An Insert (60) may be used.
